FDA labeled Stromectol for intestinal strongyloidiasis and onchocerciasis. Sarcoptes is not on that list. US clinicians still reach for oral ivermectin when permethrin fails, when crusted scabies needs systemic help, or when an outbreak logistics problem appears. That use should be spoken as off-label, not as "the Stromectol dose."
Many protocols use two swallows about a week apart because eggs hatch late. This margin will not print a homemade day-0 / day-7 calendar as if it were an FDA table. The person who saw the skin owns the dates.
Weight still matters. Borrowing four tablets from the oncho 65-84 kg band because a blog said "150" may not match the mcg/kg the clinician intended for mites. Some scabies protocols use a different mcg/kg figure than this site's 150 lock. Do not mix them on the kitchen counter.
02 The second swallow is about eggs, not about doubt
A second date exists because a single pass can miss mites that were eggs. It does not exist because the first swallow "felt weak" after pizza. Keep the empty-stomach rule both days. Do not add a third tablet on day 3 for luck.
Household contacts, bedding, and topical permethrin (when used) are the rest of the visit. Oral ivermectin without the environment work is how the itch returns and someone blames the 3 mg tablet.

11 Two dates at most unless a ward says more
Off-label, two clinician-set swallows, empty stomach, treat the house. That is the scabies ink. Not a veterinary tube. Not a weekly forever.
